Our mission at Fidelis Community Services Inc. is to support people with disabilities live independently, safely, and participate fully in their communities. We offer a wide range of services throughout Massachusetts, including:
Community Living Services
Adult Family Care: Adult Family Care (AFC) is a program that provides in-home care to individuals with disabilities. Caregivers are typically family members, friends, or neighbors who are willing to provide care to an individual with a disability. Caregivers receive a tax-free stipend, training, and support from a multidisciplinary team.
DDS Shared Living: DDS Shared Living is a program that provides shared housing to individuals with disabilities. Shared housing is a type of housing where individuals with disabilities live with other individuals who do not have disabilities. Shared housing provides individuals with disabilities with the opportunity to live independently and to build relationships with other people.
DDS/MRC Supportive Living: DDS/MRC Supportive Living is a program that provides support to individuals with disabilities who live in their own homes. Supportive living provides individuals with disabilities with the assistance they need to live independently, such as help with activities of daily living, transportation, and accessing community resources.
MRC In-Home Supports: MRC In-Home Supports is a program that provides in-home care to individuals with disabilities. Caregivers are typically certified nursing assistants or home health aides who are trained to provide care to individuals with disabilities. Caregivers provide assistance with activities of daily living, such as bathing, dressing, eating, toileting, and transferring.
Community Participation Services
Adult Day Program: Adult day programs provide a safe and supportive environment for individuals with disabilities to spend time during the day. Adult day programs offer a variety of activities, such as social interaction, recreation, and education.
Supported Employment: Supported employment is a program that helps individuals with disabilities find and maintain employment. Supported employment provides individuals with the assistance they need to find and keep a job, such as job training, job coaching, and transportation assistance.
Assistive Technology Services
Remote Support and Monitoring: This service provides individuals with disabilities with access to 24/7 support from a qualified provider. The provider can monitor the individual's health, safety, and other needs using live communication technology. This service can help individuals with disabilities live more independently and feel more confident in their ability to manage their own care.
Environmental Modifications: This service makes changes to the environment to make it more accessible for individuals with disabilities. This can include installing ramps, widening doorways, and adding grab bars. Environmental modifications can help individuals with disabilities move around their homes more easily and safely.
